Patient Information

1. Appointments

New Patients:

To make your first visit experience easier, please arrive 15 minutes prior to your scheduled appointment so that you can complete your registration. Or, to save time you may complete your new patient registration form in advance and bring it with you (Download it here). You might also want to check out our procedures and policies. Lastly, be sure to bring your insurance card and office co-pay as well as your current pair of glasses, your contact lens information, and any other pertinent documentation you may want to share.

If you are being referred by another doctor or are transferring your care to our office, please bring as much of your medical record as you are able to obtain. If you are unable to acquire your records in advance, we will be happy to assist you with the necessary record release forms during your visit.

Unless otherwise mentioned, your eyes will be dilated on your first visit. Please bring your sunglasses with you as your eyes will be sensitive to light afterwords, we also have temporary sunglasses at our front desk for your convenience.

Existing patients:

Please let the receptionist know the nature of your visit and how long it has been since you last saw us. This helps us schedule the proper amount of time and reduces the need for return visits.

If you need to cancel an appointment, please let us know as soon as possible. We appreciate at least 24 hours’ notice. We reserve the right to charge patients $35 for missed appointments without notice within 24 hours prior to the appointment.

2. Insurance Information

Participation varies as changes occur within the insurance industry. This general information is subject to change and insurance companies may not pay even if they are on the list. Subscribers must refer to their individual policies and contact their carriers for their specific eligibility.

3. Office policies

Billing Questions:

For billing questions, please call our billing office at: 301-670-4250 x 411. Please have your statement with you.

You can also email them at Please include the following information: Patient name, name of the physician, date of service, account number and phone number in the email.

It is the patients’ responsibility to request and secure a referral, if needed, prior to the appointment. Please ask your referring doctor to fax to 301-587-1269 or bring in to the appointment. For your convenience, we will file your primary insurance claims if you provide us with the correct billing information. We ask you to pay the deductible, co-insurance, or co-payment at the time of service. We accept cash, Visa and MasterCard.

Refraction Policy:

Please note that we do not accept any Vision Plans. Refraction and Contact lens fittings are generally not covered by most health insurances, we do charge for these services at the time of appointment.

4. Surgeries:

We perform surgeries at Friendship Ambulatory Surgery Center in Chevy Chase, MD. It is located at: 5550 Friendship Blvd, Suite 270, Chevy Chase, MD 20815.
