New Patients:

Thank you for choosing Silver Spring Eye. We value your time and make every attempt to see our patients in a timely fashion. You can help us in this by downloading and completing your Patient Registration Forms in advance of your visit. If you are unable to do so please arrive 15 minutes early to complete these forms in the office. Please be sure to bring your insurance cards and if you've been referred, your referral form. If you have a co-payment please bring it on the date of service.

Please bring your current eyeglasses, contact lens prescription if applicable, and any other documents that you think might assist us in your care. If you are transferring your care to our office, please bring as much of your medical record as you are able to obtain. If you are unable to acquire your records in advance, we will be happy to assist you with the necessary record release forms during your visit.

Unless otherwise mentioned, your eyes will be dilated on your first visit. Please bring your sunglasses with you as your eyes will be sensitive to light afterwards, we also have temporary sunglasses at our front desk for your convenience.

Existing patients:

When scheduling your appointment please let the receptionist know the nature of your visit and how long it has been since you last saw us. This helps us schedule the proper amount of time and reduces the need for return visits.

If you need to cancel an appointment, please let us know as soon as possible. We appreciate at least 24 hours' notice. We reserve the right to charge patients $35 for missed appointments without notice within 24 hours prior to the appointment.

Silver Spring Eye accepts Medicare as well as most major insurers in the Washington Metro area. Most insurers offer a variety of plans which provide different benefits. We ask that our patients check their plan to be aware of their benefits.

Our doctors are Medical Doctors who specialize in the diagnosis and treatment of eye diseases and perform surgery. Medical doctors bill under Medical insurance. You may have a separate "Vision Insurance" in addition to Medical Insurance. "Vision Insurance" is generally provided by doctors of Optometry who specialize in refraction and fitting of contact lenses and eyeglasses. Although we will gladly check you for eyeglasses and contact lenses, as Medical doctors we do not participate in "Vision Plans".

Please include the following information: Patient name, name of the physician, date of service, account number and phone number in the email.It is the patients' responsibility to request and secure a referral, if needed, prior to the appointment. Please ask your referring doctor to fax to 301-587-1269 or bring in to the appointment. For your convenience, we will file your primary insurance claims if you provide us with the correct billing information. We ask you to pay the deductible, co-insurance, or co-payment at the time of service. We accept cash, Visa and MasterCard.

Refraction Policy:

Please note that we do not accept any Vision Plans. Refraction and Contact lens fittings are generally not covered by most health insurances, we do charge for these services at the time of appointment.
