New Patients:

Thank you for choosing Silver Spring Eye. We value your time and make every attempt to see our patients in a timely fashion. You can help us in this by downloading and completing your Patient Registration Forms in advance of your visit. If you are unable to do so please arrive 15 minutes early to complete these forms in the office.

 Remember to Bring Your:

  • Current Insurance Cards
  • Driver’s License
  • Primary Care Referral (if required by your insurance)
  • Form of Payment

Please bring your current eyeglasses, contact lens prescription if applicable, and any other documents that you think might assist us in your care. If you are transferring your care to our office, please bring as much of your medical record as you are able to obtain. If you are unable to acquire your records in advance, we will be happy to assist you with the necessary record release forms during your visit.

Unless otherwise mentioned, your eyes will be dilated on your first visit. Please bring your sunglasses with you as your eyes will be sensitive to light afterwards, we also have temporary sunglasses at our front desk for your convenience.

Existing Patients:

When scheduling your appointment please let the receptionist know the nature of your visit and how long it has been since you last saw us. This helps us schedule the proper amount of time and reduces the need for return visits.

If you need to cancel an appointment, please let us know as soon as possible. We appreciate at least 24 hours’ notice.
